Portfolio Review Checklist
Thank you for scheduling your Home Education Annual Evaluation with ClearPath Homeschool Testing.
Your appointment will take 20–30 minutes over Zoom, and the process is simple, friendly, and stress-free. To ensure your evaluation goes smoothly, please review the checklist below and prepare the required items.

Please complete our simple Google Form — it covers your learning log, reading materials list, and all subjects in one easy checklist!
Takes just 5–10 minutes • Meets Florida’s log requirement • Makes your evaluation seamless
1. Student Information
Have this ready to confirm at the start of the meeting:
- Student full name
- Date of birth
- Grade level (approximate is fine)
- County / school district
- Date you filed your Letter of Intent (approximate is fine)
2. Log of Educational Activities + Reading Materials
Florida law requires a “log kept contemporaneously with instruction” and a list of reading materials used.

- Subjects covered (reading, math, science, etc.)
- Reading materials used (books, programs, apps)

Alternatively, you may bring a planner, calendar, notebook list, or written summary — but the Google Form is by far the easiest option and ensures you have everything needed.
3. Reading Materials Used
If you completed the Google Form above, you’re all set! If not, bring a list that includes:
- Books read or listened to
- Textbooks or curriculum titles
- Online programs, reading apps, or library books used
4. Samples of Student Work
Please prepare 3–5 samples from each main area:
Examples: Writing samples (early year + recent), comprehension pages, narrations or summaries, photos or screenshots
Examples: Worksheets, workbook pages, screenshots from online programs
Choose from: Science, Social Studies, History, Geography, Art, STEM, Electives
Samples may be:
- Photos
- PDFs
- Screenshots
- Notebook pages shown to the camera
5. Student Present for 3–7 Minutes
Florida Statute 1002.41(1)(f) requires a brief discussion with the student.
Your child will only need to answer a few simple questions, such as:
- “What was your favorite thing you learned this year?”
- “Tell me about a book or topic you enjoyed.”
- “Can you show me something you’re proud of?”
This portion is short, friendly, and pressure-free.

If you can check these boxes, you are fully prepared:
- Completed the Portfolio Learning Summary form (covers your log + reading list + subjects)
- Reading/writing work samples ready (3–5 samples)
- Math work samples ready (3–5 samples)
- Two additional subject samples ready (2–3 each)
- Child available for 3–7 minutes during the Zoom call
